Southwest General Medical Group, Inc. - Behavioral Health provides assessment, counseling, crisis intervention and self-help services. Our experienced, board-certified psychiatrists work with adolescent, adult and geriatric patients to help them deal with mental health or chemical dependency issues and life-changing events (such as the loss of a loved one or divorce).

Phone Calls

Phone calls are accepted Monday through Friday, 9 a.m. - 4:30 p.m. The office phone number is 440-816-5790. If you need to speak with a physician after hours, call our office number and leave a message with the answering service. The doctor will return your call as soon as possible. If you have an emergency, please go the closest Emergency Room or call 911!

Appointment Scheduling

Appointments are available Monday through Friday, 9 - 4:30 p.m. Call the office at 440-816-5790 to schedule an appointment. Some same-day appointments may be available.

If you need to cancel or reschedule an appointment, we require 24-hour notice so that we can use the time to accommodate other patients. Otherwise, we will need to bill you for the appointment time.

Patient Privacy/Confidentiality

All Southwest General Medical Group, Inc. practices are HIPAA compliant. All patients are asked to read our Privacy Policy.

Prescriptions and Refills

Prescription renewal requests may be made by either the patient or the pharmacist. Please fax renewal requests to our office at 440-816-5806. Although same-day renewals are available, we request at least one-day’s notice for prescription refills so that we can be sure to accommodate the needs of our patients.

The following information will be needed for the prescription request/refill:

  • Name of medication
  • Strength of medication
  • How often medication is to be taken
  • Quantity needed (30 day or 90 day)
  • Pharmacy phone number
  • Pharmacy fax number (if you are requesting that the Rx be faxed into pharmacy)
  • Phone number where the patient can be reached, in case we need to contact the patient that day

Test Results

Once your physician has received and reviewed your test results, we will contact you by phone with the results or share them with you at your next appointment. If you have questions about your test results, please contact our office. The doctor will answer your questions via telephone.

Insurance

Southwest General Medical Group physicians are approved providers for many insurance plans. New patients are asked to call the Mental Health phone number listed on their insurance card to obtain an authorization from the insurance company prior to their first appointment.

Billing/Payment Information

Patients should bring their insurance card, driver’s license and co-pay to each appointment. Co-pays are expected at the time of visit when checking in for your appointment.
