Southwest General Medical Group, Inc. – Berea Family Practice focuses on providing excellent quality medical care for family members of all ages, from newborns through seniors. Our board-certified physicians offer compassionate, comprehensive care, including preventive medical care as well as acute and chronic care services. In addition to diagnosing and treating illness, they provide routine checkups, health-risk assessments, immunizations and screening tests. Our family practice physicians provide personal care and attention to each patient and work together with patients to help them maintain a healthy lifestyle and ensure their optimal health and wellbeing.

Phone Calls

Phone calls are accepted Monday through Friday, 8:30 a.m. - 4:30 p.m. (Wednesdays until 1 p.m.)  The office phone number is 440-816-5188. If you need to speak with a physician after hours, please leave a message with the answering service to have the physician paged. The doctor will return your call as soon as possible. If you have an emergency, please go the closest Emergency Room or call 911!

Appointment Scheduling

Appointments are available as follows:

Monday, Tuesday, Thursday, Friday: 8:30 a.m. - 4:30 p.m.
Wednesday: 8:30 a.m. - 12:30 p.m.

To schedule an appointment, call the office at 440-816-5188. Same day appointments may also be available. If you need to cancel or reschedule an appointment, please call the office. We require 24-hour notice of cancellations so that we can use the time to accommodate other patients. You should bring your current insurance card and a photo ID with you to all appointments.

Patient Privacy/Confidentiality

All Southwest General Medical Group, Inc. practices are HIPAA compliant. All patients are asked to read our Privacy Policy.

Prescriptions and Refills

If you need to renew a prescription, you or your pharmacist may call our office. Pharmacists also may fax a refill request to the office. Although same-day renewals are available, we request at least one-day’s notice for prescription refills so that we can be sure to accommodate the needs of our patients.

The following information will be needed for the prescription request/refill:

  • Name of medication
  • Strength of medication
  • How often medication is to be taken
  • Quantity needed (30 day or 90 day)
  • Date on which the prescription was last filled
  • Pharmacy phone number
  • Pharmacy fax number (if you are requesting that the Rx be faxed into pharmacy)
  • Phone number where the patient can be reached, in case we need to contact the patient that day

Test Results

Once your physician has received and reviewed your test results, we will contact you by phone with the results and with any further directions/suggestions from your physician. Our goal is to get test results to you within one week if possible. If you have questions about your test results, please contact our office so that the doctor and/or staff can answer your questions.

Hospital Care and Outpatient Services

Southwest General Medical Group, Inc. physicians are affiliated with Southwest General and offer hospital care and outpatient services at the hospital’s Main Campus or at its conveniently located facilities in Brunswick, Middleburg Heights and Strongsville.

Insurance

Southwest General Medical Group, Inc. physicians are approved providers for many insurance plans. However, it is your responsibility as a patient to contact your insurance company by calling the member services phone number to verify that the physician you are scheduling with is an approved provider on your insurance plan. Please bring your current insurance card with you to your appointment along with a photo ID.

Referrals/Pre-certifications

If our office has suggested that you see a specialist, your insurance company may require a referral. It is your responsibility as a patient to determine if your insurance requires a referral, to verify that the specialist is on your plan, and to obtain a referral from our office before visiting the specialist. We require 24-hour notice, if possible, for referral requests.

Billing/Payment Information

Co-pays and any outstanding balances are expected at the time of your visit, when checking in for your appointment. Please bring your insurance card with you to each visit for verification. Please notify us of home address, phone number and/or insurance changes. This will allow for proper contact and billing.
