Brunswick Family Practice

Southwest General Medical Group, Inc. – Brunswick Family Practice provides excellent quality medical care for family members of all ages, from newborns through seniors.

Our board-certified physicians offer personal care and attention and work with patients to help them achieve optimal health and wellbeing. In addition to  diagnosing and treating illness, they provide routine checkups, health-risk assessments, immunizations and screening tests.

Southwest General's Travelers' Clinic, designated as an official Yellow Fever Vaccination Clinic by the State of Ohio, is located in the Brunswick Family Practice Office.

Phone Calls

Phone calls are accepted Monday through Friday, 8:30 a.m. - 4:30 p.m. The office phone number is 330-558-0070. If you need to speak with a physician after hours, please leave a message with the answering service to have a physician paged. The doctor will return your call. If you have an emergency, go the Emergency Room!

Appointment Scheduling

Appointments are available Monday through Friday and, from September through April, on Saturday mornings. Some same-day appointments are available. To schedule an appointment, call

330-558-0070. If you need to cancel or re-schedule an appointment, please call the office. We require 24-hour notice so that we can use the time to accommodate other patients.

Patient Privacy/Confidentiality

All Southwest General Medical Group, Inc. practices are HIPAA compliant. All patients are asked to read our Privacy Policy.

Prescriptions and Refills

To renew a prescription, contact your pharmacist who will fax your refill request to our office. We request 12 – 24-hour notice for prescription renewals so that we can be sure to accommodate the needs of our patients. Same-day renewals may be available if the request is faxed early in the day.

The following information is needed for prescription refill requests:

• Name of medication
• Strength of medication
• How often medication is to be taken
• Quantity needed (30 day or 90 day)
• Date the prescription was last filled
• Pharmacy phone number
• Pharmacy fax number (if you are requesting that the Rx be faxed into pharmacy)
• Phone number where the patient can be reached, in case we need to contact the patient that day

Test Results

You will receive test results by phone and/or on follow up visits with the physician. Our goal is to get test results to you within one week if possible, depending on the type of testing. If you have questions about your test results, please contact our office so that the doctor and/or nurse can answer your questions.

Hospital Care and Outpatient Services

Southwest General Medical Group, Inc. physicians are affiliated with Southwest General and offer hospital care and outpatient services at the hospital’s Main Campus or at its conveniently located facilities in Brunswick, Middleburg Heights and Strongsville.

Insurance

Southwest General Medical Group, Inc. physicians are approved providers for many insurance plans. However, it is your responsibility to contact your insurance company by calling the member services phone number to verify that the physician you are scheduling with is an approved provider on your insurance plan and to verify any plan limitations, co-payments and restrictions, such as which Lab must be used. Please bring your current insurance card to your appointment along with a photo ID.

Referrals/Pre-certifications

If our office has suggested that you see a specialist, your insurance company may require a referral. It is your responsibility as a patient to determine if your insurance requires a referral, to verify that the specialist is on your plan, and to obtain a referral from our office before visiting the specialist. We require one-week notice for referral requests.

Billing/Payment Information

Co pays and outstanding balances are expected at the time of your visit, when checking in for your appointment. Please bring your insurance card with you to each visit for verification. Please notify us of home address, phone number and/or insurance changes. This will allow for proper contact and billing.
