Physician Practices
Internal MedicineSouthwest General Medical Group, Inc. – Internal Medicine provides excellent quality medical care for patients aged 15 and older. Our board-certified physicians specialize in prevention, diagnosis and treatment of diseases primarily affecting adults. Internal medicine physicians have special training in the health of internal organs and disease prevention, and are skilled in treating patients who have chronic illnesses or multiple illnesses at the same time. In addition to diagnosing and treating illness, they provide routine checkups, health-risk assessments, immunizations and screening tests. Our internal medicine physicians provide individual care and attention to each patient to ensure optimal health and wellbeing. Phone CallsPhone calls are accepted during business hours as follows: Monday & Tuesday 9 a.m. - 5:30 p.m. The office phone number is 440-816-5220. If you need to speak with a physician after hours, please leave a message with the answering service to have the physician paged. The doctor will return your call. If you have an emergency, go the Emergency Room! Appointment SchedulingAppointments are available as follows: Monday & Tuesday 8:30 a.m. - 5:30 p.m. To schedule an appointment, call the office at 440-816-5220. Some same-day appointments may be available. We require 24-hour notice if you need to cancel or re-schedule an appointment so that we can use the time to accommodate other patients. There is a $25 no-show fee. Exclusions are given for urgent matters. Patients who are visiting for the first time should bring all their current medication bottles with them. Please Note: Our physicians do not prescribe chronic pain medications. Patient Privacy/ConfidentialityAll Southwest General Medical Group, Inc. practices are HIPAA compliant. All patients are asked to read our Privacy Policy. Prescriptions and RefillsIf you need to renew a prescription, please bring a list of the medications you are taking to our office at your follow-up visit. Also, you may call our office, and your pharmacist may call or fax our office, with refill requests. Although same-day renewals are available, we request at least one-day’s notice for prescription refills so that we can be sure to accommodate the needs of our patients. Physicians will not refill narcotic medications after office hours. The following information will be needed for the prescription request/refill:
Test ResultsTest results will be shared with you during your follow-up visit with your physician. In case of abnormal lab or test results, you will be called and your physician will provide any necessary directions for care. If you have questions about your test results, please contact our office so that the doctor and/or other staff can answer your questions. Hospital Care and Outpatient ServicesSouthwest General Medical Group, inc. physicians are affiliated with Southwest General and offer hospital care and outpatient services at the hospital’s Main Campus or at its conveniently located facilities in Brunswick, Middleburg Heights and Strongsville. InsuranceSouthwest General Medical Group, Inc. physicians are approved providers for many insurance plans. However, it is always in your best interest as a patient to contact your insurance company by calling the member services phone number to verify that the physician you are scheduling with is an approved provider on your insurance plan. Please bring your current insurance card with you to your appointment along with a photo ID, such as your driver’s license. Referrals/Pre-certificationsIf you need to see a specialist, your insurance company may require a referral. It is your responsibility as a patient to determine if your insurance requires a referral, to verify that the specialist is on your plan, and to obtain a referral from our office before visiting the specialist. For referral requests, we request notice one to two weeks in advance of your visit with the specialist. Billing/Payment InformationCo-pays and any outstanding balances are expected at the time of your visit, when checking in for your appointment. Cash, check, MasterCard or Visa are accepted for payment. Patient with self-pay accounts must make payment in full at the time of their visit. Please bring your insurance card with you to each visit for verification. Please notify us of home address, phone number and/or insurance changes. This will allow for proper contact and billing.
